The Indiana Minority Health Coalition’s Healthy Lifestyles Program helps individuals and families take control of their health by providing the knowledge, tools, and support needed to live well. From chronic disease prevention to daily wellness strategies, our programs are built to reduce health disparities and foster long-term healthy habits—especially in communities of color.

National Diabetes Prevention Program (NDPP)
An evidence-based lifestyle change program proven to prevent or delay type 2 diabetes.

The Healthy Lifestyles Program at the Indiana Minority Health Coalition empowers individuals and families to make informed choices for better health. Through education, support, and community-driven initiatives, we help prevent chronic conditions like diabetes and heart disease—promoting wellness through programs such as the National Diabetes Prevention Program, Diabetes Self-Management & Support, and holistic Self-Care workshops.
